UPDATE 13 NOV 2016:
I still don't owned Skyrim SE so if anybody knows how to port this armor, you may do as you wish.
First, I wanna thanks CD Projekt RED for allowing use of their assets.
This mod will add craftable Kaer Morhen armour which initially use by Geralt in Witcher 3. It is originally Russian ver (found in gamer-mods.ru). The problem with Russian version is the usage of too many material when crafting and upgrading (40 leather to upgrade 1 piece of armor? now thats madness).
I already tweaked the value and material usage.
NOTE
Since this model ported from Witcher 3, it doesnt have a female version and slider support. Considering how bulky Geralt is, best if you play your character with 100 weight.
This mod also comes with a dark retexture

Since Ciri's armor's been converted over to Skyrim, and seeing Yen and Triss's outfits have been converted as well, that got me thinking. Why aren't there any more Witcher 3 armor's converted over? Particularly the Witcher Gear like the bear armor. This has been done many times over with assets from the Witcher 2 and some of those mods are the most popular on Nexus. If anyone's up to it I'd bet money that the mod witch does this would become one of the most popular as well. In the same way that Ciri's armor was though. I know Geralt's default armor was made as well but not to the same quality standard. Just an idea.
Skyrim Witcher 3 Armor Mods

The Witcher 3 has some amazing looking armor that Geralt can wear and even some awesome NPC ones. CDPR also allows assets to be used from their games.. So.. Where are the Witcher 3 armor mods?
I have the Yennefer, Triss, and Ciri armors which prove how good Witcher 3 armor looks in Skyrim. I never play females characters though so I just give them to any followers I have.
I almost can't believe this doesn't exist. I keep typing it in on nexus expecting the results to suddenly appear in front of me. I don't have the skill to do this. Maybe someone reading this does?

The Witcher 3 is a fun, beautiful, lonely game. Though Geralt’s story intersects with dozens of lively NPCs, more fleshed out than their counterparts in other RPGs, most of his travels are alone with only his horse, Roach, at his side. You’ll fight with many of your allies (most notably at an enormous battle at Kaer Morhen), but after that? Everyone more or less goes their separate ways.A new mod for The Witcher 3 is changing that, and has the capacity to make the game feel a lot more like Skyrim, where you’re almost always being followed by a companion, unless you choose to dismiss them and go solo.
You need thesethreemods in order to make this work. A video of them in action is below, so you can see how it appears in the game. A host of companion characters are available to choose from, including young Ciri, shown in the video, and you’ll also recognize a bonus feature of the mod as you’re watching.
This game was released on 23rd January 2007 and Paradox Interactive published this game. Europa universalis 3 download.
A new, non-Geralt hero!
The player here is using Triss Merigold, strapped with Geralt’s swords as their main character. There are a few NPCs you can sub in here, so you can wander around the countryside as someone other than Geralt and Geralt alone.
Obviously, there are limitations to this mod. Unlike Skyrim, you’re not going to be able to give your companion your extra gear to “share your burdens,” as Lydia might say. Brother sewing machine parts. Nor will you be able to have voiced conversations with them. And it goes without saying that if you’re suddenly playing as Triss or Yennefer or Shani, Geralt’s lines won’t be magically re-recorded in the game’s numerous cutscenes.
I did feel a bit lonely playing through The Witcher 3 as Geralt, only interacting with others during specific missions. But the concept of a companion doesn’t really work well within the context of the game. Character interactions are pretty complex, so it would have been weird to drag Triss along on Yennefer missions, and having Ciri as a companion would be downright impossible most times as you’re spending the majority of the game looking for her. I think a companion system could have worked with a few select characters (Zoltan is one of the only ones that comes to mind), but I think the game was right to avoid it, even if it’s a popular concept in games like Skyrim or Dragon Age.
But in the world of mods? Go crazy. Sure, it won’t make any plot sense, but the ability to play as multiple NPCs and use other NPCs as your followers seems like a lot of fun. Give it a try for yourself and see how your new heroes and companions perform in combat.
